Patient case management software: what it needs

What patient case management software does, the features to require, the main types of platform, and how to build your own on a HIPAA-eligible plan.

Short answer: patient case management software coordinates a patient's care across people and services, from intake and assessment to a care plan, tasks, referrals, follow-ups and closure. Every case has an owner, every next step has a due date, and everyone involved (case managers, supervisors, providers, referral partners and the patient) sees only their own slice. To be usable with real patients it also needs role-based access enforced on the server, an audit trail and a vendor that signs a Business Associate Agreement. This guide covers what the software has to do, the features to require, the main types of platform, and how to build your own on Tadabase AI from the Patient Case Management & Care Navigation template.

What is patient case management software?

It is the system a care team uses to run a caseload. Where an EHR records clinical facts about a visit, case management software tracks the journey: what has to happen next for this patient, who owns it, by when, and what came of it. Health systems, home health agencies, behavioral health providers, care navigation programs, social services and nonprofits all use it. Day to day it handles:

  • Intake and assessments: a new case opened from a referral, a call or an online form, with the first assessment attached.
  • Care plans and task tracking: the goals for the case and the tasks that move it forward, each with an owner and a due date.
  • Referrals and follow-ups: sent to a provider or agency, accepted or declined, completed, with the outcome recorded.
  • Scheduling and reminders: check-ins and visits, and reminders that fire on their own.
  • Compliance documentation: consents, documents, and a record of who did what.
  • Communication and reporting: notes on one timeline per case, and caseload and outcome reports for supervisors and funders.

The case lifecycle the software has to support

Whatever the program, a case moves through the same stages. Good software makes each stage a status with rules attached, not a column someone remembers to update.

StageWhat happensWhat the software should do
Referral and intakeA case is opened from a partner referral, a call or a formCreate the case, assign a case manager, start the program's checklist of first tasks
AssessmentNeeds, risks and eligibility are recordedAttach the assessment to the case and set priority
Care planGoals and the services that will meet themTurn the plan into tasks with owners and due dates
CoordinationReferrals out, appointments, documents inHold referrals until consent is on file; track each referral to an outcome
Follow-upCheck-ins and progress notesRemind the owner before a task is due, flag it when it slips
EscalationA case stalls or misses its target dateNotify the supervisor, then the next level, automatically
ClosureGoals met, patient transferred or disengagedRecord the reason, summarize the case, keep the history read-only

What to look for in 2026

These are the non-negotiables. Use the checklist when you sit through a demo or scope your own build.

Patient case management platforms: the main types

Most products fall into one of four groups. The right one depends on how standard your process is and who outside your team has to take part.

TypeBest forStrengthWatch out for
Case management module in an EHRClinics where every participant already has an EHR loginClinical record and case in one placeRigid workflows; outside partners and patients rarely get access
Specialist case management softwarePrograms that match the vendor's model (behavioral health, nurse case management, value-based care)Built for the domain, with its reportsLimited customization, quote-based pricing, long onboarding
A CRM adapted for careTeams that already run a CRMFamiliar tool, good contact historyBuilt for sales relationships, not care processes, consents and outcomes
A custom app on a platformPrograms with their own process, partners and reportingFits your workflow exactly, portals for every roleYou configure it, so you need a fast way to build and change it

Custom development used to mean a software project with developers. That is the part that has changed: an AI app builder can produce the case app from a description or a template, and your team keeps changing it afterwards.

CRM vs case management software

A CRM tracks relationships, usually with customers or donors: contacts, deals, emails. Case management tracks a care process over time: the plan, the tasks, the referrals, the consents and the outcome, with access rules tied to who is involved in each case. You can bend a CRM into a case tracker, but you end up rebuilding the parts that matter most for care (consent gates, caseload-scoped access, escalation and an audit trail) on top of a tool not designed for them.

Build it on Tadabase AI from the template

Tadabase AI is not an EHR or an out-of-the-box CRM. It builds the case management app your program actually runs: describe it in plain English, or start from the template at tadabase.com/templates/patient-case-management-care-navigation. The template installs with:

  • 7 roles: Admin, Supervisor, Navigator, Provider, Patient, Referral Partner, and a login-free Partner Intake link that returns only a reference number.
  • 12 tables (Patients, Cases, Notes, Tasks, Referrals, Documents, Consents, Audit Log, Organizations, Care Programs, Providers, Referral Partners) with row-level security on every one.
  • 18 workflows: case intake, reassignment, task reminders and overdue flags, the escalation ladder, consent gates on referrals and documents, consent expiry warnings, partner intake and audit entries.
  • 16 email templates for case, task, referral, consent and escalation notices.
  • 8 AI agents, including Intake Triage, Stalled Case Monitor, Case Summary & Handoff and a Patient Message Drafter.
  • Optional sample data: demo users for every role and seeded patients, cases and referrals.

Then make it yours: rename the care programs and set the checklist each new case starts with, add the fields your assessments record, set reminder and escalation timing to your standard of care, and invite your team, providers and referral partners. App users are unlimited on every plan, so partners and patients do not add per-seat cost.

Teams build the same pattern for home health coordination, referral and triage, discharge planning and follow-up, behavioral health treatment tracking, care team scheduling and documentation, and data collection for audits and grants.

HIPAA considerations

Case notes, referrals and documents about identifiable patients are protected health information. Tadabase AI is HIPAA-eligible: on HIPAA-eligible plans we sign a BAA and run your app on BAA-covered AWS infrastructure. For a case management app that means:

  • HIPAA is a switch on the app, including one built from the template. You flag which tables and fields hold PHI.
  • Row-level security is enforced on the server, so the rule that limits a navigator to their caseload also covers the API and the front end.
  • A PHI access log records reads and changes of PHI with the user, action, table, record, source IP and result, never the values.
  • HIPAA setup applies automatic logoff, two-factor authentication for end users and a password policy.
  • A publish gate checks PHI tables, deny-by-default row-level security, secure file fields, access logging and BAA-covered hosting before every publish, and blocks a configuration that fails.

Buy or build?

  • Buy a specialist product when your program matches its model closely, everyone involved works inside your organization, and you are happy to adopt its reports.
  • Use your EHR's module when the case work is clinical and stays inside one clinic.
  • Build on a platform when your process is your own, outside providers, partners or patients need their own logins, funders want specific reports, or per-user pricing makes a specialist tool expensive at your scale.

Common questions

What is case management in healthcare? The coordination of a patient's care across services and providers so they get the right care at the right time with efficient use of resources: assessment, planning, coordination, follow-up and review.

What is the difference between CRM and case management software? A CRM tracks relationships, typically sales or donors. Case management software tracks care processes, consents, compliance and outcomes over time.

Is Tadabase a patient case management tool? Tadabase AI is a platform you build one on, tailored to your workflows, data, users and reporting. The Patient Case Management & Care Navigation template gets you a working app to start from.

Can patients and outside providers log in? Yes. In the template, patients see their own case status, timeline and shared documents and can upload documents; providers see only the referrals sent to them and record outcomes; referral partners see their own referrals.

Does it cost extra per case manager or partner? No. App users are unlimited on every Tadabase AI plan, and there is a free plan with no card to try it. Storing PHI requires a HIPAA-eligible plan.

Can we change the workflow later? Yes. Describe the change in plain words and the builder makes it, or edit the tables, roles and workflows yourself. Schema changes on a live app show a plan and wait for your approval.

Where to start

Write down your case stages, who owns the next step at each one, and who outside your team needs to see what. Then copy the Patient Case Management & Care Navigation template, rename the programs, and run a week of real cases on sample data before inviting providers, partners and patients. If the data is PHI, turn HIPAA on and clear the publish gate before the first real patient goes in.
