Employee performance tracking template. Free, and ready to become an app.
One workbook for performance reviews: every employee with their job title, department and manager, each review with its date, reviewer, overall score, status and comments, the criteria you score against and how much each one weighs, the tasks a review cycle creates, and a log of the reminders sent. Download it free. When managers, employees and HR all work the same reviews, describe the same structure to Tadabase AI and it builds the review app, with each employee seeing only their own reviews and each manager only their team.
Excel · Google Sheets · Numbers · 6 sheets
Rather have it as an app? Describe it — we’ll build it now.
Same structure as the spreadsheet, plus logins, roles and workflows. Free to start.
6 sheets, already linked by ID.
Each sheet is one kind of record. The ID columns are how the sheets point at each other, and they are the same links the app version turns into real relationships.
-

-
Reports
A log of the summaries you produce, such as the quarterly performance summary and the review completion status.

Notification ID · Recipient · Type · Message · Date Sent · Status
Fine for one person. Hard for a team.
Where the spreadsheet starts to cost you time once more people depend on it.
-
The weights never reach the score
Review Criteria gives teamwork, problem solving and punctuality a weight, but a review has one Overall Score typed in by hand. Nothing scores a review criterion by criterion, so the weights describe the method without applying it.
-
Everyone with the file sees every review
Scores and comments for the whole company sit in one workbook. There is no way to let an employee open only their own review, or a manager only their team, short of keeping separate copies that drift apart.
-
The Notifications sheet sends nothing
It records that a reminder was delivered after someone sent it by hand. A task due tomorrow or a review still marked Scheduled after its date does not remind anyone.
Same structure, with logins and automations.
Start from a real prompt, change anything that doesn’t fit, then build it.
Loads this prompt into the prompt box- Performance review app An employee performance review app with employees, managers, review cycles, review criteria with weights, and reviews. A manager scores each of their direct reports on every criterion and adds comments; the overall score is calculated from the criteria scores and their weights. Employees log in and see only their own reviews. Managers see only their own team. HR sees everyone, schedules each review cycle and has a dashboard of completed and pending reviews by department. The employee gets an email when their review is completed.
- Review reminder workflow A review reminder workflow — every review has a due date and a reviewer; each morning, reviewers with a review due in the next three days get an email listing them; reviews still not completed after their due date are flagged for HR; and every review task shows who it is assigned to, its priority and when it was finished.
- Goals and check-ins tracker An employee goals tracker — each employee has goals for the quarter with a target and a due date; they post progress updates with a date and a comment; their manager can reply to each update; employees see only their own goals; and managers see a dashboard of their team's goals with the latest update on each.

Questions about this template.
Is the spreadsheet template really free?
Yes. It is a normal .xlsx file that opens in Excel, Google Sheets or Numbers. No sign-up is needed to download it.
Does the template calculate a weighted score?
No. The Review Criteria sheet lists the weights, but the Overall Score on each review is typed in. In the app, each criterion gets its own score and the overall score is calculated from them, so the weights apply to every review.
Can each employee see only their own review?
In the app, yes. The AI sets up an employee role that sees only that person's reviews and goals, a manager role that sees their direct reports and an HR role that sees everyone. That rule is enforced on the server for every record, not by hiding columns.
Can I bring the data I already have in the spreadsheet?
Yes. Once the app is built, import each sheet into the matching table from a CSV or Excel file. The importer maps your columns to the fields and shows any row it could not bring in, and why.
When should I move from the spreadsheet to an app?
When more than one person edits it, when someone outside your team needs to see part of it, or when you start copying rows between tabs by hand. Those are the points where a spreadsheet starts losing data quietly.
Keep the structure. Lose the copy-paste.
